I certify that I am not a fugitive from justice, or under indictment. I have not been convicted of a felony, misdemeanor or any crime. I am not drug or alcohol dependent; I am not under adjudication of mental incompetence; I have not been convicted for domestic violence.
I acknowledge the risks and dangers that exist in my use of any and all firearms and/or defensive tactics and techniques.
I expressly agree and promise to accept all of the risks existing in this activity. Participation in this activity is purely voluntary, and I elect to participate in spite of the risks.
I agree when arriving or departing range property, I must keep all firearm(s) unloaded and either cased or actions open.
Students agree to follow all safety rules as outlined by the instructor and class materials.
I shall immediately notify an instructor(s) of any injury that I receive, or should I observe injury to any other participant.
Class Policies
If, for any reason the course is cancelled the student will be notified by e-mail or phone or text. Class can be cancelled at company's discretion.
In the event of class cancellation, tuition may be rolled over to the next available class date at the discretion of management.
Conditions, such as, weather, improper equipment, closed range, traffic conditions and emergencies may influence the completion date of a class.
Certification is not guaranteed by paying tuition and attendance. Course standards must be met.
Upon successful completion of the program certificates will be provided within 30 days to the person or entity paying for the program.
Professional individuals who attend our programs under guidance from their organization agree to pay for their tuition should their organization default on payment. Ultimately the license certificate belongs to the individual.
Classes, Range Sessions, and events can and will be held in other locations other than the corporate office as designated by management.
Company reserves the right to ask any to remove themselves from our Class Room or the Shooting Range immediately.
Waiver & Release of Liability
I am fully aware of the risks and hazards connected with the participation in this course, including physical injury or even death, and hereby elect to voluntarily participate in said event, knowing that the associated physical activity may be hazardous to me and my property.
I HEREBY ASSUME ALL OF THE RISK OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risk that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that to the best of my knowledge, I am in good physical condition and fully able to participate in this course and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems, which preclude my participation in this course. If you are pregnant; please understand due to safety concerns, we cannot offer you the shooting portion of the range, but we will be happy to reschedule your class as needed.
I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or loss or damage to property owned by me, as a result of participation in this course.
I RELEASE, WAIVE, DISCHARGE, from any and all liability, including but not limited to, liability arising from negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this course. THE FOLLOWING ENTITIES OR PERSONS: USS Academy LLC, their officers, servants, agents, and employees (hereinafter referred to as RELEASEES).
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this course or activity.
I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the Entities Company, subsidiaries, executives, employees and associates mentioned in this waiver from any and all liabilities, claims, loss, damage occurred, or caused as a result of participation or attendance in this course, program, or event.
I understand that during this course or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by USS Academy LLC, their officers, servants or agents.
